Please utilize this Examination Booklet as a "Work Sheet" for arriving at the correct answer to each question.

When you have completed answering the questions in this Booklet, carefully transfer your answers to the ANSWER SHEETS found at the end of this Booklet.

Please print your answers neatly and clearly.  If the Examination Auditors can not understand what your answer is to any question, it will be marked as "incorrect."

Make certain you have printed your name and I.M.S. Membership Number at the top of each Answer Sheet.

Carefully fill-out the COVER SHEET that must be sent along with your Answer Sheets.
